Innovations in Practice: Collaborative Care Models

The success of health care reform depends in part upon the success of new, collaborative models of care in which physical therapists and other health care providers deliver seamless, coordinated, and evidence-based care. Physical therapists are helping to improve health care outcomes, control costs, and improve the health of our population through successful involvement with these models. In this session, attendees will learn about the innovative involvement of physical therapists in new models of health care delivery and payment.
