Course Information

Nearly 1 in 5 Medicare patients discharged from a hospital is readmitted within 30 days, often for avoidable reasons. Viewed as a measure of your facility's quality of care (or worse, its failure), the ripple effects are staggering. Explore the costs and regulatory challenges associated with avoidable hospital readmissions, and discover why physical therapists are positioned to be the right provider at the right time to minimize their occurrence while improving patient-centered care and adding value to your facility. Attendees will take away strategies to help them:

  1. Recognize the value physical therapists bring to the discharge planning process and postacute follow up, and apply best practices and proven strategies to circumvent them.
  2. Identify the 3 key conditions the Centers for Medicare and Medicaid Services will track for all readmitted Medicare patients beginning October 2012.
  3. Minimize unplanned hospital readmissions and improve patient-centered care.

Presenter Bio(s)

Anita Bemis-Dougherty, PT, DPT, MAS, received a Bachelor of Science degree in Physical Therapy at The University of Maryland, Baltimore, MD, Masters degree in Administration from the Johns Hopkins University, Baltimore, Maryland, and Doctorate in Physical Therapy from Widener University in Pennsylvania.

Bemis-Dougherty has worked in tertiary, academic medical institutions which included a variety of clinical settings including outpatient, acute care, and acute inpatient rehabilitation. Previously, she was the Physical Therapy Manager at the Johns Hopkins Hospital in Baltimore, Maryland. She joined the staff of the American Physical Therapy Association (APTA) in June, 2006 and is currently an Associate Director in the Department of Practice. She has been involved in the APTA initiatives related to physical therapy inpatient initiatives, physical therapy documentation, Peer Review, fall risk reduction and prevention, acute care initiatives, revisions to the Guide Physical to Therapist Practice, and education of members in the application of the International Classification of Functioning, Disability, and Health (ICF).


Mary Fran Delaune PT, MPT, MPH, is the Director of the Practice Department for the American Physical Therapy Association. In this position, she provides consultation to other APTA departments and members, liaisons with various external organizations and contributes to policy development to advance the practice of physical therapy. Currently she acts as project manager for the revisions to the Guide to Physical Therapist Practice, the National Outcomes Database, Innovations in Physical Therapist Practice and Evidence Based Documents.

Prior to joining staff at APTA, Delaune spent fourteen years in direct clinic practice and practice administration. Her primary areas of clinical expertise are in orthopedics and acute care. She earned a BS degree in Health Sciences and a Master of Physical Therapy degree at the University of Sciences in Philadelphia and a Master of Public Health at the George Washington University.

On successful completion of the examination, 0.2 CEUs (2 contact hours/CCUs) and a certificate of completion will be awarded. The American Physical Therapy Association has been approved as an Authorized Provider by the International Association for Continuing Education and Training (IACET), 1760 Old Meadow Road, Suite 500, McLean, VA 22102. Provider (#380).
