Course Information

Innovation Summit 2013: Collaborative Care Models is a live 1-day event to attend virtually. It will bring together leaders in the health care profession and, specifically, the physical therapy profession to explore integrated models of care such as ACOs, PCMHs, and bundled payment, and the role of physical therapy, with the goal of developing strategies for bolstering the involvement of physical therapists in these models.

Participants will be able to watch live as their colleagues, physicians, large health systems, and policy makers who are actively participating in and at the decision-making helm in these models discuss the role of physical therapy and the future of health care.

Presenter Biographies


Learning Objectives
  1. Identify the latest developments in the formation of integrated delivery systems under the Affordable Care Act.

  2. Identify trends in private payer and employer-sponsored plans regarding direct access and increased focus on the value of physical therapy.

  3. Differentiate between ACOs, bundled payment models, and patient-centered medical homes and address the challenges and opportunities posed by each for physical therapists.

  4. Assess current implementation of innovative models under Medicare, Medicaid, and commercial insurers and implement successful strategies to participate in these programs.

  5. Apply new administrative and clinical processes to collect outcome data and form relationships with large health systems and physician groups to improve quality and lower costs.

  6. Adapt your practices to address clinical and regulatory challenges and opportunities that arise regarding the implementation of innovative models of care such as ACOs, PCMHs, and bundled payment models.

  7. Implement quality improvement, patient-centered, and population health initiatives into your practice and articulate your practice's value in integrated models of care.

Course Completion

In order to complete this course you should:

  • Complete the practice benchmark evaluation to assess your current practices.
  • View the Innovation Summit panel discussions.
  • Create an action plan for incorporating innovative models of care into your practice.
  • Once you have successfully completed the action plan, you will be prompted to Claim Credit.
  • In order to Claim Credit, you'll be prompted to complete a post course evaluation. 
  • Submit the evaluation and then you will be able to print your CEU certificate for 0.9 CEUs (9.0 contact hours/CCUs).
  • In 3 months, complete the post evaluation in the APTA Learning Center to document your progress on your goals in order to receive an additional 0.1 CEU (1.0 contact hour/CCU) credit.

On successful completion of the practice benchmark, action plan, and 3 month post evaluation, you will earn 1.0 CEUs (10 contact hours/CCUs) and a certificate of completion will be awarded. The American Physical Therapy Association has been approved as an Authorized Provider by the International Association for Continuing Education and Training (IACET), 1760 Old Meadow Road, Suite 500, McLean, VA 22102. Provider (#380).
