This course is designed for physical therapy professionals seeking to enhance their understanding of pain management across the continuum of care, with a particular focus on transitions from acute care to inpatient rehabilitation. It examines critical gaps in current practice, including the overreliance on opioid-based treatments and the frequent omission of psychological contributors to pain. Learners will explore evidence-based, multimodal pain management strategies that integrate physical, psychological, and rehabilitative approaches to support patient-centered care. The course highlights the essential role of inpatient rehabilitation as a bridge between acute care and community reintegration. Through case studies and literature review, participants will gain practical tools to incorporate rehabilitation-based pain management into clinical workflows. Emphasis is also placed on reducing readmissions, supporting long-term recovery, and improving patient outcomes through self-management strategies and individualized discharge planning.
