Through this experience she has been able to collaborate with world-renowned surgeons and physicians including those who work with the NFL and MLB. Sara has also been a contributor to many popular publications such as US News & World Report, MSN, Spine Universe, and Cosmo Magazine. Sara opened her practice in 2016 to offer a more holistic approach to healthcare. She believes in providing lifestyle medicine to improve health -- treating and focusing on exercise, diet, sleep, and stress relief. She is an adjunct professor at Touro University Manhattan and Tufts University Seattle and is an official Spokesperson for the American Physical Therapy Association Media Corps.
Dr. Marko graduated with a Master's Degree in Physical Therapy from Simmons University in Boston and obtained her Doctorate Degree in Physical Therapy from Des Moines University in Iowa. Since 2013, inflation has risen by over 32%. This means what took $1.00 to purchase an item in 2013 now takes $1.32 to purchase the same item in 2024. Unfortunately, insurance reimbursement has not risen 32% over the same 11-year period. In fact, considering inflation, you are actually being paid less in 2024 than you were in 2013, yet your expenses, especially health insurance, have increased drastically. For example, the Medicare conversion factor has decreased 9% since 2018.
This inverse relationship between reimbursement and cost to run a practice is fueling the trend for providers to go out of network (OON). But what does being out of network mean for your billing practices? Many providers go out of network only to continue running their practice as if they were in-network – writing off denied and unpaid claims, accepting arbitrary reimbursement cuts, and failing to balance bill patients. Providers must change their mindset and business practices to have a successful OON practice.
This session will discuss considerations for going out of network, including whether you can have some therapists stay in-network while others are out of network, the pros and cons of accepting or not accepting assignment, and how to balance bill patients and carriers for OON claims. In addition, the session will teach participants what they must still do each visit regarding documentation and billing even though they have no contractual relationship with the insurance carrier. The session will discuss being out-of-network with Medicare Advantage (MA) plans and financial limitations.Learning Objectives:

- Provide a method for decision-making when analyzing whether to go out of network with commercial carriers and Medicare Advantage Plans.
- Discuss the pros and cons of accepting assignment on out of network claims.
- Outline the differences in how in-network and out of network claims are (and should be) managed depending on whether the provider accepts assignment or not.
- Discuss prompt pay discounts, balance billing patients and managing claims denials/payment reductions.
- Summarize what information an insurance carrier may or may not be able to share with you (and vice versa) if you have no contractual relationship with that insurance carrier

Join payment advocacy experts as they provide an update on cutting-edge and timely issues. Topics will include the annual Medicare update focused on the fee schedule, 2025 annual therapy dollar threshold, 2025 Medicare Part B deductible, telehealth, physical therapist assistant supervision, new, revised, and deleted CPT codes for 2025, and the 2025 Merit-Based Incentive Payment System (MIPS) program. In addition, this session will discuss new issues for 2025 related to the implementation of new Medicare Advantage and other CMS policies that hold the promise to decrease some common administrative burdens, the evolution of the Payment Consortium, and the payment wins chalked up by PT advocates across the country, as well as a review of payment advocacy tools like the APTA Value of PT Report and an update of the APTA Private Practice Direct to Employer Services Designation Program.

Robert Hall

Bob Hall works for the Private Practice Section by serving as the Senior Consultant handling payment issues. He helps health professionals maximize their collective impact as Executive Chair of a solo consulting firm focused on improving access to quality, affordable health care. From 2016-2018, Bob was the Director of Government Relations at the American Academy of Family Physicians. Bob directed all legislative and regulatory advocacy for the AAFP, including changes to the Medicare fee schedule’s treatment of primary care and oversight of the Academy's Center for State Policy. Bob was also elected to represent AAFP as the first Co-Chair of the Partnership for Medicaid, a nonpartisan, nationwide coalition made up of 23 organizations representing doctors, health care providers, safety net health plans, counties, and labor. Bob spearheaded the access to care portfolio at the American Academy of Pediatrics from 2006-2016, leading efforts to improve insurance coverage for children. Prior to his tenure at AAP, he served as Director of Government Relations for the National Coalition for Cancer Survivorship, Counsel to US Senator Mark Dayton, Attorney Advisor to the US Securities and Exchange Commission and Chief Clerk to the Texas House of Representatives Insurance Committee. He holds a Masters in Public Affairs from the LBJ School of Public Affairs, a Juris Doctor from the University of Texas School of Law and a BA from Haverford College.

As physical therapy businesses, we are faced with the reality of continuously tightening margins. Our payment landscape is challenging and our labor and fixed costs keep going up. For these reasons and more, we often focus on the measures that we can control and that help us keep our doors open - largely financial, operational, and patient experience metrics. In light of our current labor market, the continued shortage of PTs in our workforce, and what we know see, and feel as the result of burnout in our profession, we need to look at more than our operational financial and patient quality measures in order to sustain. We must find ways to measure, track, analyze, and improve the experience of our employees. While measuring something as mushy as joy in work can seem idealistic at best and unreliable at worst, there are validated ways to do it, with potentially positive results to gain. If you want to explore how to retain and engage your employees in a meaningful and sustainable way, measuring joy in work may be an answer.

From 2011-2023, physical therapy reimbursement declined 37.0%. In 2024, Medicare reimbursement is scheduled to decrease by another 3.4%. Healthcare providers and insurance companies are looking at other reimbursement and care delivery models as the way of the future. Value-based care is that model. Medicare has clearly stated that all payments to healthcare providers will be in a value-based care model by 2030. Other insurance companies will follow. The clock is ticking for physical therapy providers to prepare for this seismic reimbursement and care delivery shift. Given the magnitude of this change, physical therapy providers will not be able to simply “flip the switch”. In this session, you will learn from two pioneers in the value-based care area. They were talking about value-based care long before it was fashionable. Marc Gruner is a Mayo Clinic fellowship-trained physician with extensive clinical experience in developing value-based care models. He is also the founder and Chief Medical Officer for Limber Health. Kennedy Hawkins was instrumental in commercializing the Intermountain ROMS clinical outcomes platform, which is built upon value-based care models.
