This course focuses on improving the effectiveness of home exercise programs for individuals recovering from stroke, especially during the transition from clinic‑based care to at‑home rehabilitation. Learners will explore common challenges that affect safety, adherence, and outcomes, and gain practical strategies to support patients outside the clinic.
Topics include adapting exercises to individual needs, motivational interviewing, and integrating telehealth check‑ins to monitor progress and make timely adjustments.

Dr. Catherine Lewan, PT, DPT, C-IAYT, earned her Doctorate in Physical Therapy from the University of Illinois Chicago in 2010 and a BA from Wake Forest University in 2001. A former professional dancer, she transitioned to physical therapy after a career in fitness and health. Dr. Lewan’s first PT position was in an orthopedic clinic, where she launched an abdominopelvic PT program that expanded to include three pelvic health PTs, whom she mentored and managed. With over 15 years of experience treating complex orthopedic, neurological, pelvic, and dysautonomia cases, she relocated to Charleston and expanded her work as a research PT and adjunct faculty. Dr. Lewan’s clinical and research interests focus on individualized care, autonomic regulation, chronic pain, abdominopelvic PT, pain science education, connective tissue disorders, TBI and empowering patients in self-management. She is certified in Pilates mat and apparatus as well as yoga therapy, with additional 300-hour yoga and 85-hour prenatal yoga training. She owns Move Well Collective, a trauma-informed, biopsychosocially-focused clinic, where she sees clients and offers small group classes. Dr. Lewan also develops and teaches evidence-based continuing education courses for healthcare professionals, emphasizing a person-centered approach to care.
